The above information must be completed upon registration. A NON-REFUNDABLE registration fee of $200EC is applicable. All costumes MUST be paid for in full by May 31, 2009. Submitting this form waives all rights to claims for use of photographic or video images of oneself in any promotional media.

Loss of receipt results in the forfeiture of prior payments. Initial payments (registration fees) are to be made to your section leaders. Receipts of prior payments are to be taken for the making of additional payments.
